چکیده

A 68-year-old man without any structural heart disease underwent ablation of persistent atrial fibrillation in 2008 (isolation of 4 pulmonary veins and electrogram-based ablation of the posteroinferior left atrium) and 2009 (left pulmonary vein reisolation and linear ablation of the left atrial [LA] roof with demonstration of bidirectional block). Recently, he was referred for the ablation of recurrent sustained atrial tachycardia (AT) with stable cycle length of 230 ms (Fig. 1A). A steerable decapolar catheter was inserted into the coronary si-

Perimitral atrial flutter associated with a protected coronary sinus after a Maze IV procedure and concomitant mitral annulus repair

Introduction The Maze IV procedure during cardiac surgery is an established treatment for patients with atrial fibrillation (AF) concomitant with a mitral valve repair or replacement. Recurrent atrial tachyarrhythmias are mainly left atrial (LA) flutter, which is associated with gaps on previous surgical lesions. Herein, we report a case in which a 3-dimensional mapping system could demonstrate...

Bipolar radiofrequency catheter ablation for refractory perimitral flutter: a case report

BACKGROUND Mitral isthmus is often targeted as a part of stepwise approach during radiofrequency ablation for persistent atrial ablation. Acute success rate in achieving the mitral isthmus block is only modest, late reconduction rate is relatively high and, consequently, incomplete lesion may be proarrhythmic. We describe the first-in-man experience with successful MI ablation by bipolar RF ene...

Transthoracic epicardial ablation of mitral isthmus for treatment of recurrent perimitral flutter.

BACKGROUND Perimitral flutter (PMF) is a common form of left atrial tachycardia after atrial fibrillation (AF) ablation. The mitral isthmus (MI) is the standard ablation target. However, in some cases bidirectional block cannot be achieved. OBJECTIVE The purpose of this study was to describe the first experience using a transthoracic epicardial (TTE) approach to treat recurrent PMF after prio...

Efficacy of ablation at the anteroseptal line for the treatment of perimitral flutter.

BACKGROUND Left atrial flutter following atrial fibrillation (AF) ablation is increasingly common and difficult to treat. We evaluated the safety and efficacy of ablation of the anteroseptal line connecting the right superior pulmonary vein (RSPV) to the anteroseptal mitral annulus (MA) for the treatment of perimitral flutter (PMF). METHODS We systematically studied patients who were previous...

Different patterns of interatrial conduction in clockwise and counterclockwise atrial flutter.

BACKGROUND The terms counterclockwise (CC) and clockwise (C) atrial flutter (Afl) are used to describe right atrial activation around the tricuspid valve in the left anterior oblique view. The manner in which the left atrium is activated, as reflected by coronary sinus (CS) recordings, has not been systematically evaluated. METHODS AND RESULTS Nine patients with both CC and C Afl underwent el...
